By Now You’ve Heard … Yes, Strathmoor Pediatrics will be closing it’s doors soon. We are no longer accepting new patients. It is with a sad heart that I must inform you, I will be closing the doors of Strathmoor Pediatrics as of 31 May, 2014. This office has been such a big part of my life for the past 15 years and I will miss all the wonderful children and families that I have come to know over the years. I must inform you that you will need to find another pediatrician to continue your child’s medical care. There are many fine physicians in the community, so I am unable to single out individuals for you. Please be sure to choose someone that accepts your child’s medical insurance. Also, please insure you have refilled all chronic medications prior to 31 May 2014.
Obtaining Records … I ask that you do not take your records home, but that you have them sent to a medical facility to prevent them from getting lost. Please do not call the office for the records. The website will remain in effect until July, 2015 in order to more easily accommodate everyone. Please download HERE and complete a Record’s Request form or come into the office and complete a form in person. The form must be complete to include FULL name, Address, and Telephone Number and Fax Number of the Facility Receiving the Medical Record. There MUST be a separate request for EACH child. Incomplete requests will not be honored. The form file is in PDF format and can be accessed with a PDF viewer, such as Acrobat Reader, which is free to download from the Adobe website. The files on the compact disc are also PDF format – If a facility cannot read a disc, we encourage you to print the files in question. Most facilities also limit the file size of faxes received, so we will not fax records larger than 35 pages.
Fees … The request MUST be signed and dated by an authorized caregiver, not a relative or friend. You are entitled to pick up a copy of the medical records from the office at 2211 Meadow Drive, Louisville, KY 40218 without charge. There will be a fee to receive medical records by fax*, or mail. Please be mindful that to minimize costs large files will be placed on a compact disc. Any record sent by fax (*more than 35 pages), will incur a fee of $5 to be paid prior to sending the record and will be sent only if the receiving facility agrees to the transmission. Any record that is mailed and is less than 10 pages will incur a $5 charge. Any record that is mailed and is more than 10 pages and less than 50 pages will incur a $10 charge. Any record that is mailed, over 50 pages and less than 100 pages will incur a $20 charge. Any record over 100 pages that is mailed will incur a $30 charge. To mail a compact disc, the charge remains at $5. I apologize for any inconvenience this may impose on you. Rest assured. we are doing all we can to ease the transition. You may pay in person with cash, debit card, credit card or by Paypal.
